Your Visit

Please refer to the guidelines below to ensure that you are prepared for your first visit to Northwest ENT and Allergy Center.

Protecting you is our priority:

We allow one person to accompany you into the exam room during your appointment unless an aerosol-producing procedure is performed. In this case, no one can be present other than you and the Physician.
If you have multiple people with you, they must remain in the waiting room for the duration of your visit.

Please allow at least one hour for your initial appointment.

If an emergency arises, we will do our best to notify you before your appointment; however, please be aware that is not always possible.

Pre-Appointment Checklist
Completed patient information forms Name, address, and phone number of your pharmacy Insurance card(s) Driver’s license Any previous medical records, especially test results and X-rays (if needed)

Please arrive thirty minutes before your appointment if you need to complete your paperwork at the office.
